We have an experienced staff waiting to care for your loved one and offer you both the love and support needed to overcome challenges you may be facing. We will perform a free assessment and coordination of services which include over 50 years of senior adult experience and care. We believe in a supportive, healthy environment for our seniors. If you have a Inpatient Physical Rehab or Behavioral Health need that TrustPoint can help with, please allow us to come do a free assessment. You will be asked for certain medical information such as:
- Patient Face Sheet (if available)
- Patient H&P (if available)
- Patient Medication List
- Most recent labs.
- An assessment will be initiated to determine eligibility and admission criteria.
- TrustPoint Social Services will arrange transportation for eligible rehab and behavioral health patients
